چکیده

BACKGROUND Vascular access options in coronary angiography can be considered a preference-sensitive decision, where the benefits/risks have different levels of significance, depending on the individual patient. For preference-sensitive healthcare options, patient decision aids (PtDA) significantly improve the process of decision-making. The purpose of this trial was to evaluate the effectiveness of an evidence-based PtDA compared with usual care in patients eligible for radial and femoral artery access. METHODS AND RESULTS We conducted a single-center, nonblinded, randomized controlled trial with patients eligible for both femoral and radial access as per their treating physician. The PtDA was designed to guide patients to make an informed choice, consistent with their preferences and values. The primary outcome, decisional conflict, was assessed using the validated decisional conflict scale. One hundred fifty patients were randomized (vascular access PtDA=76 versus usual care=74). The intervention group had a significantly reduced decisional conflict scale compared with control (unadjusted 14.8 versus 19.5, P=0.04) and were significantly more knowledgeable regarding risks/benefits associated with each vascular access (mean knowledge score 3/5 (95% confidence interval, 2.6 to 3.3) versus 2/5 (95% confidence interval, 1.7 to 2.3, P<0.01). PtDA patients had better informed value congruence with their vascular access received (47.3% versus 25.7%, P<0.01). There were no significant differences in procedural success or safety between the 2 groups. CONCLUSIONS A vascular access PtDA for eligible patients undergoing coronary angiogram procedures reduces decisional conflict and improves value congruence and the patients' knowledge of their healthcare options; however, a multicenter study, powered to confirm these benefits and evaluate differences in procedural success or complications, is required.

Effect of Acupressure on Low Back Pain and Fatigue after Coronary Artery Angioplasty through Femoral Artery: A Randomized Controlled Clinical Trial

Background and purpose: Complaints of low back pain and fatigue caused by prolonged lying on the back are complications of angioplasty. The aim of this study was to evaluate the effect of acupressure on severity of low back pain and fatigue in patients after coronary artery angioplasty through femoral artery. Materials and methods: A simple randomized controlled clinical trial was carried out ...

A comparative review of various interventional methods for coronary angiography and angioplasty

Background and Objective: Coronary artery disease (CAD) is one of the most common diseases today. Among the diagnostic methods, angiography is the main and gold standard in the diagnosis Since angiography is an invasive procedure, this procedure complications range widely from minor problems with short term sequelae to life threatening ones may cause irreversible damage or even death. Materials...

Moderate procedural sedation and opioid analgesia during transradial coronary interventions to prevent spasm: a prospective randomized study.

OBJECTIVES The aim of this study was to test the hypothesis that moderate procedural sedation can reduce the incidence of radial artery spasm. BACKGROUND Transradial access for left heart catheterization and percutaneous coronary intervention is increasingly used for emergent and elective procedures, in lieu of the femoral approach. However, increased rates of access site crossover have been ...

Radial Versus Femoral Access for Coronary Angiography/Intervention in Women With Acute Coronary Syndromes: Insights From the RIVAL Trial (Radial Vs femorAL access for coronary intervention).

OBJECTIVES The purpose of this study was to determine the efficacy and safety of radial versus femoral access in women undergoing coronary angiography/intervention. BACKGROUND The risk of bleeding and vascular access site complications are higher in women than in men. METHODS In a pre-specified RIVAL (RadIal Vs femorAL access for coronary intervention) subgroup analysis, we compared outcome...

Analysis of safety outcomes for radial versus femoral access for percutaneous coronary intervention from a large clinical registry

OBJECTIVE Using a multisite, contemporary registry of 58 862 percutaneous coronary intervention (PCI) procedures in a national healthcare system, the present study compared radial access with femoral access on safety and efficacy outcomes. METHODS This is a real-world, large-scale, retrospective study using clinical data from a 137-hopsital System and reported to a multisite clinical registry...
